Request for Change in Academic Catalog

Note: All currently enrolled students will continue to be assigned to and follow the provisions of their catalog of entry into the University or the catalog in effect during a change in their declaration of degree or major. Only students completing this form will officially transfer to the provisions of the most recent Academic Catalog which includes the revised General Education Program. Furthermore, transferring to the most recent Academic Catalog may also change course and GPA requirements for their program(s) of study. Students should consult the Registrar’s Office or their adviser of records to determine the most viable option for their academic plan of study and catalog assignment.
